The slope intercept form of a line is
y = mx + b
I am going to guess that by "standard form" you mean
y  y_{1} = m (x x_{1}) where
m = ( y_{2}  y_{1})/(x_{2}  x_{1})
I am going to proceed with an example.
I want to find the equation of the line that passes through (3,2) and (1,4). The standard form gives
m = ( 4  2 )/(1  3 ) = (6)/(2) = 3
Thus the line in standard form is
y  2 = 3(x  3) or
y 2 = 3x  9
which simplifies to
y = 3x  7
